Physical Health Matters Too - A Community and Mental Health Planning & service Coordination Initiative

North East PCP and Hume Whittlesea PCP are working with Area Mental Health (AMH), Psychiatric Disability Rehabilitation Support Services (PDRSS), Community Health (CH) and Divisions of General Practice to address the physical health gap between people with mental illness and the general population.  By supporting the introduction of physical health screening in mental health services it is anticipated that the physical health needs of consumers will be increasingly identified and appropriately followed up through referrals to services such as community health and general practice.

The following resources have been developed as a result of the initiative:

NEPCP and HWPCP engaged Rob Ryan from Wisdom + Work to undertake and independent evaluation of this project aimed to gather qualitative and quantitative data to describe the impact and outcomes of physical health screening. The evaluation notes that: success factors for the Project included the high quality of the PCP leadership, the appointment of a Project Coordinator and the high engagement of the partners.
